Legal name, the name they actually go by, birth date, and the room or schedule they are enrolling in.
Parents and household
Both parents or guardians, addresses, work numbers, and a line for who to call first during the day.
Authorized pickups
Names, relationship, phone, and a check box for ID verified. This is the page you will reach for most.
Health and allergies
Provider, conditions, allergies with reaction and response, current medications, and immunization record status.
Emergency consent
Permission to seek emergency care, plus the hospital preference some families want noted.

Enrollment that does not drag on for a month
Most enrollment friction is process, not paperwork. The packet works best when there is a clear moment where it gets handed over and a clear moment where it comes back.
Give the whole packet at the tour. One envelope, one due date. Drip-feeding forms is what turns a two-day process into three weeks.
Set a start-day rule. Health page and signature page complete before the first day, everything else within a week. Say it out loud at the tour.
File it the day it comes back. Check for blanks while the family is still standing there. A missing allergy line found in March is a real problem.

What has to be on file before a child's first day?
This differs by state, and the common core is emergency contacts, health information, immunization records, and signed consents. Get your own state's required-records list and check the packet against it before you use it.
Do both parents need to sign?
Depends on your policy and any custody arrangement. Many programs require the enrolling parent to sign and ask for court documentation when pickup is restricted.
Can we collect this online instead?
Yes, and it is faster. Confirm whether your licensor accepts electronic signatures for the required consents, and keep a printable copy for the file either way.
How long do we keep records after a child leaves?
Ask your licensor. Programs often keep enrollment records for a set period after departure, and a labeled box of former families is worth the shelf space.
